Le travail est pénible, pourtant Manson est bien décidé à accomplir au mieux sa mission : soigner. Des lugubres mines galloises aux quartiers chics de Londres, il lui faudra résister à l'incurie d'un système médical corrompu qui mettra à mal son intégrité.
A sa parution en 1937, La Citadelle rencontra un énorme succès. Dans ce texte largement autobiographique, A. J. Cronin (1896-1981) trouve le parfait équilibre entre divertissement romanesque et critique sociale. On considère aujourd'hui qu'il a joué un rôle dans la création du National Health Service, l'équivalent britannique de la sécurité sociale. Un livre qui ne cherche pas autre chose que faire naître l'émotion et soulever l'indignation.
